[Board] Reinstatement options poll, reformatted

1) No children as reinstatements.  They should apply and stand on their own
2) Deceased partner..no change
3) Split partners...one gets the membership...the other is on their own to
apply.  Eliminate automatic membership provision
    Joining at the same time should remain in section 4.  The last sentence
should be eliminated.
4) Reinstatement.  Change to 5 years.  If it is after that, they reapply in
the normal manner.
5) Reinstatement options....maximum of 2 per year.
