Looks great so far.
Minor spelling error on #8 - "Events" for children
As far as #11 is concerned we may simply want to re-word it as "Explore the possibilities of increasing the number of members"
In addition we may want to add an option to #11 that relates to the exploration of other revenue generating ideas, such as increasing the club rental fee or adding a soda machine.
In general I think this is a great start and think we can use these multiple times per year to gauge member's feelings on any subject.

OK, I've finally got to making the PYC member survey form. In discussions with many board members, it was the consensus that anyone who wanted to give a response should be willing to give their name. It was felt that anonymous responses might lead to some stuffing of the ballot box and although I implemented cookies to prevent people from entering duplicate surveys, it can be done if people have multiple computers, so I put name as a required field.
I've played with it a bit, but would appreciate your feedback. It can be accessed here:

Remember that it's going to count your responses, so make them accurate.
