[Board] Member Survey

OK, I've finally got to making the PYC member survey form.    In discussions
with many board members, it was the consensus that anyone who wanted to give
a response should be willing to give their name.   It was felt that
anonymous responses might lead to some stuffing of the ballot box and
although I implemented cookies to prevent people from entering duplicate
surveys, it can be done if people have multiple computers, so I put name as
a required field.

 

I've played with it a bit, but would appreciate your feedback.   It can be

Remember that it's going to count your responses, so make them accurate.
