Hi Dan,

 

The survey looks good. I like the anonymous responses and single response
per membership ideas.

 

Would a question asking, how often do you bring guests to club events, be
appropriate, for this round?

I think the question "what do you not like about the club" very negative.
Is there another way of phrasing it that is not so open ended?
